By providing the above usernames, passwords, and any other credentials (collectively, “Credentials”), you authorize Phone Data Extraction, LLC (“PDE”) to use the Credentials to access your devices, online accounts, and any associated data strictly for the purpose of digital forensic analysis and evidence retrieval as directed by you and your attorney. You confirm that you have full legal authority to provide these Credentials and grant PDE permission to perform whatever actions it deems necessary to obtain the requested information, including using APIs or downloading data from third‑party platforms.

You acknowledge that you have reviewed your other online accounts (such as banking, email, and social media) to determine if any of them use the same passwords as your Google or Apple accounts. If so, you understand that it may be prudent to change those passwords before submitting this form so that PDE does not inadvertently receive access to unrelated accounts. PDE will not be responsible for any access to or use of other accounts that share the same credentials.

You agree that PDE, its owners, employees, affiliates, and vendors will not be liable for any loss, damage, unauthorized access, or legal claim arising from or related to the use of the Credentials for the purposes described above. You release and hold harmless PDE from any and all claims related to hacking, unauthorized use, device or data corruption, or any other incident that may occur during PDE’s possession or use of the Credentials, except to the extent caused by PDE’s gross negligence or willful misconduct.
